Labile variations of intraventricular conduction unrelated to rate changes.

چکیده
An example of unstable intraventricular conduction is presented. Several unusual features of this type of conduction are described. These are: (1) variable intraventricular conduction which was sensitive to multiple stimuli but not clearly related to changes in heart rate, (2) mid and terminal QRS changes reflected only in the left lateral chest leads and in the mid and terminal QRS portions of the vectorcardiogram, (3) constant association of prolonged P-R interval suggesting that this is a variety of bilateral bundlebranch block, and (4) apparent persistence of the peculiar intraventricular conduction for at least 4 years. Multiple involvement of the conduction system is probably present. and functional variations in distal portions of the left ventricular conduction system account for these labile changes. Predictable QRS patterns can be produced by various stimuli which alter intraventricular conduction.

Rate-dependent changes in excitability of depressed cardiac Purkinje fibers as a mechanism of intermittent bundle branch block.
When the heart rate is accelerated, rate-dependent intraventricular block may occur. This block has been attributed to abnormal action potential prolongation in a diseased conducting pathway. Less often, intraventricular block develops during slowing of the heart rate and has been explained in terms of phase 4 depolarization in potentially automatic cells within the diseased fascicle. We tested...
متن کاملSoil properties, labile pools of soil organic carbon and their variations under broadleaf and coniferous plantation in Hyrcanian forest, northern Iran
Afforestation, as a tool to mitigate carbon emission is constrained by available land areain several countries, but Iran has the potential of plantation. In doing so, differences in soilstocks between tree species could give an indication of the effects of future managementchanges. Hence, a better understanding of tree species traits on soil properties is required topredict how changes in ecosy...
متن کاملThe electrophysiologic effects of lidocaine in patients with intraventricular conduction defects.
A 6 mg/kg lidocaine infusion was administered (over 22 min) to 10 patients with the electrocardiographic findings of bundle branch block. His bundle electrograms, heart rate, blood pressure, and plasma lidocaine levels were obtained before, every 5 min during, and for 20 min after the infusion was terminated. Control atrioventricular nodal conduction time (AH) was prolonged in two patients (130...
متن کاملElectrophysiologic effects of quinidine. Studies using chronically implanted electrodes in awake dogs with and without cardiac denervation.
Experiments were performed on awake dogs with chronic recording electrodes implanted on the SA node, His bundle, and right bundle branch. Quinidine at serum concentrations of 5 to 10 mg/ liter had little effect on AV conduction, but slowed conduction in Purkinje tissue, prolonged ventricular activation, decreased the excitability of atrium and ventricle, and prolonged the atrial refractory peri...
متن کاملAtriobiventricular pacing improves exercise capacity in patients with heart failure and intraventricular conduction delay.
OBJECTIVES We sought to assess the efficacy of biventricular pacing with respect to both peak and submaximal measures of exercise in patients with New York Heart Association class III heart failure (HF) and intraventricular conduction delay in a randomized, blinded study. BACKGROUND Submaximal and maximal changes in exercise capacity need evaluating in this patient population with this novel ...
